Disabled Parking Explanation

1. To qualify

• You must have a permit (disabled placard) from the State of Michigan Secretary of State.

• Your Parking Citation (Ticket)

• Both placard holder and vehicle owner must sign form.

2. Gather the following

• Placard from Secretary of State

• The parking ticket (citation)

• The form from the City Treasurer's Office

4. Submit your form

You can mail, drop off, email or fax this form using the contact information on the right.

We also have a drop box available if you are driving downtown. This is located in front of 300 Ottawa Ave NW just south of Michigan Street.

5. The Treasurer's office will review the form.

We will review the form for completion and possible void. We will only contact you if more information is needed.
