Apply for an Operational Permit
Do you own a business that uses materials that could be dangerous. Or do you own a business that holds a large number of people. You may need to apply for an Operational Permit.
1. Before you apply
Be sure to know what materials, operations, and assembly types require a Operational Permit. You can review the requirements on our guide page. You will be required in this form to identify all operations that require a permit.
Also have your all business information ready. Including your Federal ID and State ID numbers.

3. Next Steps
We will review your application once you submit it. Depending on your responses, a on site inspection may be needed. An inspector will contact you if this is the case to make an appointment. We will send you an invoice after our review. We issue the permit once we have received payment. Operational Permits must be renewed each year. The permit cycle is from January to December. We will send out a notice when it is time to renew your permit.
