Step 1.Make sure your application is eligible

If you need to renew your passport, you don't need to come to our office to apply. You can review how to Renew a Passport for more instructions.

In person submissions are required for new passport applicants and minors under age 16.

Step 2.Complete the passport application form

Fill out your application before you come to your appointment. Complete and print the passport application form and bring it to our office. This application cannot be submitted online.

When filing out application by hand, use black ink only. Make sure it's legible and avoid spelling errors.

Please do not sign the application until you are in front of a passport acceptance agent.

Step 3.Call to schedule your appointment with the Clerk's Office

Call 616-456-3010 to schedule an appointment.

APPOINTMENTS AVAILABLE MONDAY - FRIDAY

Note that we only offer appointments Monday - Friday. If you request a weekend appointment, we will deny your request.

Our appointment times are in 30-minute increments starting at 8:00 am. The last appointment time is 3:00 pm.

Step 4.Gather required documentation

First Time Applicants

  • Proof of citizenship (original or certified copy of birth certificate, naturalization papers, etc.)
  • Photo identification (driver's license, state-issued ID card)
  • If a minor child, under 16 years of age, both parents/guardians and the minor child must be present. Proof of relationship to minor child is required (birth certificate, court order, etc.)
  • Passport photo that meets the passport State Department Passport Photo Requirements. We do offer passport photo services.

Step 5.Be ready to pay your fees

PAYABLE TO THE US DEPARTMENT OF STATE

  • Adult Passport Book (10-year validity) - $130
  • Adult Passport Card (10-year validity) - $30
  • Minor Passport Book (5-year validity) - $100
  • Minor Passport Card (5-year validity) - $15
  • Expedite fee - $60

The Department of State does not accept credit or debit card payments. Here are their accepted payment types:

  • Checks (personal, certified, cashier's, or traveler's)
  • Money orders

PAYABLE TO THE GRAND RAPIDS CITY TREASURER

  • Processing fee per passport application - $35
  • Photo (taken in our office) per person - $10

The City Clerk's Office does accept cash and credit or debit cards for City fees. Here's a look at our accepted payment types:

  • Cash
  • Credit or debit card
  • Checks (personal, certified, cashier's, or traveler's)
  • Money Orders

We can only accept passport applications in person and by appointment only.

We're open Monday through Friday. Our normal passport services hours are 8:00 am - 3:45 pm. We offer Extended Passport Services Hours on select dates.

Visit us on the 2nd floor of City Hall during your scheduled appointment time. You'll find our address in the Contact Us section of this page. Here's what will happen at your appointment:

  • You'll sign your application
  • We'll take your picture
  • We'll accept payment for all of your fees
