1. Create a Citizen Access account
We use a third party application called Citizen Access for our Property Use Verification (PUV) forms. If you've never used Citizen Access, we recommend that you review how to create an account before starting your form.
2. Make sure you have time to apply
This form will take about 10 minutes once you've gathered the required information identified in the steps below.
3. Make sure you have all the required information
Gather the finer details about the property, including:
- Number of occupied units on the property (required)
- Number of vacant units on the property (required)
- Number of parking spaces on the property (required)
- Digital/scanned documents – such as utility bills – that may help to demonstrate the property use (optional)
- Template letter, such as a Rebuild Letter, Zoning Compliance Letter, or Zoning Verification Letter (optional)
4. Decide on regular (Level I or Level II) or expedited PUV
You may have a time sensitive project that requires quicker delivery. We offer two PUV findings delivery options, regular and expedited. Each option has its own fee amount.
- Regular Level I – Simple determination of legal property use - PUV findings delivered within 1 to 2 weeks
- Regular Level II – Complex determination of legal property use - PUV findings delivered within 1 to 2 weeks
- Expedited – PUV findings delivered within 1 business day
5. Have your credit/debit card ready
You'll need to enter your credit/debit card information to pay the PUV fee. We accept Visa, MasterCard and Discover.
6. Complete the online form
Login to Citizen Access and choose the Property Use Verification category using the link below.
7. We'll review your form
Expect to hear from us within 1-2 business days. We'll email you to confirm receipt of your form. Keep in mind that you might need to revise your form or attachments if we notice any missing or incorrect information.
