• A copy of your State of Michigan license

Mail or drop off the form to the address listed in the Contact Us section of this page.

Step 4.We'll review your information

We'll update your license information within 1 week of receipt of your information form. Keep in mind that you might need to revise your form or attachments if we notice any missing or incorrect information.
